[Urgent] Having a hard time to apply an associative model to a 3D grid.

Hello. I have created a model in grasshopper and now I want it to be applied to the center points of the sub-surfaces of a new 3D geometry. After plugging it in, the list structure changed and I couldn't figure it out how can I re-organize the list system again... 

I have uploaded the files. "Middle.gh" is the successful model I want to apply to another geometry. "Middle44.gh" is the failed one.

Would someone have time to help me take a look of what is going wrong in my work please? Thank you!!!!!!!!!!!!!!!
