Does anyone knows how to add description in the group like the one in the picture. I find it very useful if I do it as an exercise and review it sometimes...

Sorry somehow pictures couldnt uploaded. The description I mensioned is like a conversation cloud.

If it's what I think you're talking about just right click on the group and a context menu will open.  Type in your comments in the entry box at the top.
