Cull Duplicate Points dies when mode is set to average

Hi David,

I've recently encountered this bug in cull duplicate points. It does not always occur, but when it does it is set to average.

I'm currently in 0.9.0064, and know that I'm a bit behind in the upgrade paths - I could not find any notice of it in recent changelogs, or bug reports.

Attached is a file with internalized data that should be able to reproduce the problem.

Found and fixed the problem. The next release will work again. 

The issue was that if the first two points in a list were coincident, it would fail.

Cool, thanks for the diagnosis, this is is probably fixable or at least workaroundable for me.
