Image Sampler Perforation

Hello all: 

I have Rhino 5 with what I think is the newest version of Grasshopper. I am new to GH, and don't know how to use it very well. I am however, familiar with the interface and very basic operators. I have a client who is interested in incroporating perforated panels into the design of their Retail Store and they described something they've seen that would be easiest made in GH rather than modeled in Rhino. 

I have determined that I need to use an Image Sampler and create perforations using other operators.  Frankly, this is a bit complicated for a newbie like me. I have looked into this issue for the better half of this day. From all the files I have downloaded, not one file allows me to just substitute the vertical surface in my Rhino model and the image I input into the Image Sampler. Is there anyone here who would be willing to help me out by uploading a GH definition that works?
